How to use your Rx plan

You will receive one ID card with your Aetna Medical plan on the front and Magellan Rx plan on the back. Present your Magellan information along with your prescription to any participating pharmacy every time you fill your prescription.

A health savings account (HSA) is a medical savings account. You will earn $0.50 on every $1 you set aside to a maximum Niagara Match of $250 individual / $500 family.

Scholarship Program

Make college affordable

The Andrew D. Peykoff, Sr. Scholarship Program offers tuition assistance to dependents of Niagara Team Members who are continuing their education in college or vocational school programs. The application window is from December 1, 2019 to March 1, 2020. Recipients are selected based on academic performance, school and community involvement, statement of academic and career goals and more. The Program is administered by Scholarship America, the nation’s largest designer and manager of scholarship, tuition assistance and other education support programs for corporations, foundations, associations and individuals.

Basic Life

Team Member Supp Life and AD&D

Current Team Members

You may increase coverage by $10,000. Your total coverage amount can be up to six times your base pay up to a maximum of $300,000, without being subject to Evidence of Insurability (EOI). Amounts elected above $300,000 up to the total maximum of $1,000,000 are subject to EOI.

Newly-Hired Team Members

You may elect coverage in $10,000 increments up to a maximum of six times your base pay up to $300,000, guaranteed issue. Amounts above $300,000 up to the total maximum of $1,000,000 are subject to EOI.

Dependent Life and AD&D

Current Team Members: Up to 50% of Team Member coverage in increments of $10,000 up to a maximum of $250,000. Any amounts over $50,000 for New Hires or $10,000 during a change at Open Enrollment are subject to EOI.

Child Life and AD&D

Current Team Members: Up to $10,000.

Short-Term Disability

Provides you a portion of your paycheck if you become ill or injured and cannot work.

All active, Full-Time Team Members are automatically enrolled in STD at no additional cost, provided they reside in a state other than those that have state-mandated disability benefits (CA, NY, NJ, RI, HI and PR).

Team Members who are eligible for state-mandated disability benefits will be covered under those state plans for STD. State-mandated STD benefits will vary by state. Please refer to each state’s disability website for additional information.

Receive 60% of your weekly pre-disability earnings to a maximum benefit of $1,500 per week.

FMLA

Life doesn't always give you a heads up, and sometimes you have no choice but to face a challenge head-on.

This law, and it’s associated regulations, entitle eligible employees of covered employers to take unpaid, job-protected leave for specified family and medical reasons. The employee must be restored to employment and be provided with continuation of group health insurance coverage under the same terms and conditions as if the employee had not taken leave.

Voluntary Long-Term Disability

LTD benefits pick up where STD leaves off. Your monthly benefit amount is calculated based on your pre-disability earnings (including regular wages and commissions) but does not include awards, bonuses, overtime pay or employer contributions to any deferred compensation arrangement or pension plan.

Receive up to 60% of monthly pre-disability earnings to a maximum benefit of $15,000 per month.

NIAGARA WATER permits INSURERS to offer employees of NIAGARA WATER certain voluntary insurance programs. Whether you choose to enroll in any of these programs is completely optional and voluntary. NIAGARA WATER does not make a contribution towards the cost of these programs and employees pay the full cost of premiums on an after-tax basis. NIAGARA WATER does not sponsor, maintain, endorse, recommend, or promote these voluntary programs. NIAGARA WATER'S involvement regarding these voluntary insurance programs is strictly limited to allowing the insurer access to employees to publicize these programs and NIAGARA WATER may perform certain ministerial functions such as payroll deduction and forwarding employee premium payments to the insurer. NIAGARA WATER does not receive any consideration in the form of cash or otherwise in connection with the program, other than reasonable compensation, excluding any profit, for administrative services actually rendered in connection with payroll deductions. Accordingly, these voluntary insurance programs are not subject to ERISA and related regulations. All questions or claims regarding these programs should be directed to the insurer.

The Niagara 401(k) Plan.

Retirement Plan through Fidelity

The Niagara 401(k) Retirement Plan through Fidelity Investments helps you save for a secure and well-funded future through pre-tax or after-tax (Roth) contributions and a company match. You are eligible to participate in the 401(k) plan on the first of the month after 30 days of active, full-time employment, as long as you are at least 18 years of age. You can make changes to your plan at any time during the year by visiting Fidelity's website.

Niagara Matches your Contributions

Don't miss out on free money!

Niagara matches your pre-tax and Roth contributions starting first of the month following 30 days of service.

You are required to contribute to the plan to receive match. The company matches $0.33 of every $1 you contribute up to 6% of your gross pay. To maximize your match, contribute at least 6% each pay period.

The match vesting schedule is a 3-year cliff vesting (0%, 0%, 100%). That means on a Team Member’s 3rd Anniversary with Niagara, all past and future matching contributions become 100% vested. Team members hired prior to 2019 will retain their 20% vesting. On their 3rd anniversary, they will be 100% vested.

Don’t forget, your personal contributions to the 401(k) are always 100% vested!
