Current Team Members: You may increase coverage by $10,000. Your total coverage amount can be up to six times your base pay up to a maximum of $300,000, without being subject to Evidence of Insurability (EOI). Amounts elected above $300,000 up to the total maximum of $1,000,000 are subject to EOI.

Newly Hired Team Members: You may elect coverage in $10,000 increments up to a maximum of six times your base pay up to $300,000, guaranteed issue. Amounts above $300,000 up to the total maximum of $1,000,000 are subject to EOI.

Dependent Life and AD&D

Spouse Life and AD&D: Up to 50% of Team Member coverage in increments of $10,000 up to a maximum of $250,000. Any amounts over $50,000 for New Hires or $10,000 during a change at Open Enrollment are subject to EOI.

Guaranteed Issue

Information

Guaranteed issue is the amount of insurance you may purchase for yourself and/or your spouse without having to submit Evidence of Insurability (EOI). This means you will be guaranteed up to 6x your base pay, with a maximum limit of $300,000 of life insurance without any medical exams or questionnaires, during your New Hire enrollment period only.

If you choose to elect coverage at a later date, or increase your amount above the 6x base pay/$300,000, your coverage will be subject to EOI; a medical exam, questionnaires and the underwriting process.

Disability Plans

Employer-Paid Short-Term Disability

Information

All active, Full-Time Team Members are automatically enrolled in STD at no additional cost, provided they reside in a state other than those that have state-mandated disability benefits (CA, NY, NJ, RI, HI and PR).

GREAT NEWS! Aetna Short-term disability will be provided at no cost to all team members in states without a state disability plan (all except CA/NJ) starting in January.

Voluntary Long-Term Disability

$15,000/month maximum

LTD benefits pick up where STD leaves off. Your monthly benefit amount is calculated based on your pre-disability earnings (including regular wages and commissions) but does not include awards, bonuses, overtime pay or employer contributions to any deferred compensation arrangement or pension plan.

LTD Pre-Existing Conditions
You will be considered to have a pre-existing condition if: You received medical treatment, consultation, care or services, including diagnostic measures, or were prescribed drugs or medicines in the three months just prior to your effective date of coverage; or The disability begins in the first three months after your effective date of coverage unless you have been treatment-free for 12 consecutive months after your effective date of coverage.

The Niagara 401(k) Retirement Plan through Fidelity Investments helps you save for a secure and well-funded future through pre-tax or after-tax (Roth) contributions and a company match. You are eligible to participate in the 401(k) plan on the first of the month after 30 days of active, full-time employment, as long as you are at least 18 years of age. You can make changes to your plan at any time during the year by visiting Fidelity's website.

• The 2019 IRS Maximum contribution for pre-tax and Roth is $19,000
• The 2019 IRS Maximum for Catch-up Contributions is $6,000

Matching Contributions

Free Money!

Niagara matches your pre-tax and Roth contributions starting first of the month following 30 days of service. You are required to contribute to the plan to receive match. The company matches $0.33 of every $1 you contribute up to 6% of your gross pay. To maximize your match, contribute at least 6% each pay period.

Starting in 2019, the match vesting schedule is a 3-year cliff vesting (0%, 0%, 100%). That means on a Team Member’s 3rd Anniversary with Niagara, all past and future matching contributions become 100% vested. Team members with 2 years of service will retain their 20% vesting. On their 3rd anniversary, they will be 100% vested. Don’t forget, your personal contributions to the 401(k) are always 100% vested!
